Smithfield Court Community Rehabilitation Service

Smithfield Court is an integrated service with Social Care and Wellbeing, NHS Grampian and Housing. We provide rehabilitation, assessment, care and confidence-building activities to individuals for varying periods of time. This is generally between 4 to 8 weeks. The activities aimed at promoting independence are provided in 20 self-contained flats that are based within a sheltered housing complex. The team comprises of Social Care and Wellbeing staff, therapy staff from NHS Grampian, Care Management, with access to the community pharmacy and dietician.

Referrals to Smithfield Court Community Rehabilitation Service can be made via telephone from any professional (e.g. Hospital Staff including Liaison Nurses, GPs, care managers, occupational therapists, nurses or social work staff). Service users can be admitted from the community as well as hospital. Following the telephone referral, a team assessment is carried out by 2 members of the Smithfield Court multi-disciplinary team to determine if Smithfield Court is appropriate for the client's needs.
