Horizons Rehabilitation Centre
Horizons Rehabilitation Centre provides an assessment and therapeutic rehabilitation service for individuals with complex needs, whose disability requires a multi-disciplinary approach.The aim of the Rehabilitation Centre is to meet both the needs of people who have been physically disabled because of illness or injury, their family and carers. For the most part, Horizon offers time-limited rehabilitation programmes to enable individuals to achieve their goals and to gain maximum independence within the community. Horizons can provide transport in certain circumstances.
The main conditions seen within the Centre are of a neurological nature such as head injury, multiple sclerosis, Parkinson's disease, stroke etc, but also include individuals with complex physical disability that is not neurological in nature.
